Vláda v Uttarakhande bude distribuovať antiparazitárne liečivo ivermektín medzi obyvateľov štátu ako preventívny liek proti šíreniu látky Covid-19, uviedol vysoký úradník.

Oznámenie vlády Uttarakhandu prichádza po tom, čo Goa a Karnataka vydali podobné pokyny.

Rozhodnutie bolo urobené na odporúčanie klinickej technickej komisie na štátnej úrovni podľa príkazu hlavného tajomníka Om Prakasha všetkým okresným sudcom.

12 mg tablety lieku budú podľa objednávky distribuované všetkým rodinám v súprave prostredníctvom zdravotného oddelenia a okresného sudcu.

Dospelí a osoby staršie ako 15 rokov musia zvyčajne užívať tablety ivermektínu dvakrát denne počas troch dní po raňajkách a večeri. Jedna osoba teda potrebuje šesť tabliet a štvorčlenná rodina potrebuje 24 tabliet. Každá súprava preto bude obsahovať dve desiatky tabliet, uvádza sa v objednávke.

Deti vo veku od 10 do 15 rokov užívajú iba jednu tabletu denne, zatiaľ čo deti vo veku od 2 do 10 rokov môžu liek užívať iba po konzultácii s lekárom. Tableta sa nemá podávať deťom do dvoch rokov, tehotným ženám a ľuďom s ochorením pečene.

Karnataka

V utorok podpredseda vlády v Karnataku a vedúci pracovnej skupiny Covid v štáte Ashwath Narayan uviedol, že bolo obstaraných 10 tabliet ivhektínu lakh a ich dodanie sa začne 14. mája.

The Uttarakhand government will be distributing Ivermectin, an antiparasitic drug, among the residents of the state as a preventive medicine against the spread of Covid-19, a senior official said.

The Uttarakhand government’s announcement comes after Goa and Karnataka issued similar directions.

The decision was taken on the recommendation of the state-level clinical technical committee, an order issued by Chief Secretary Om Prakash to all district magistrates said.

The panel has recommended the Ivermectin tablet as “mass chemoprophylaxis” to effectively control the surge of Covid-19 infection apart from the vaccination drive, the order said.

The 12 mg tablets of the drug will be distributed in a kit to all families through the health department and district magistrates, the order said.

Dosage

Usually, Ivermectin tablets have to be taken by adults and those above 15 years twice daily for three days after breakfast and dinner. One person will thus need six tablets and a family of four will need 24 tablets. Hence, each kit will contain two dozen tablets, the order said.

Children between 10-15 years will take only one tablet daily whereas those aged between 2 and 10 years can be administered the drug only after doctor's advice. The tablet cannot be given to children below two years, pregnant women and those suffering from liver diseases, it said.

The kit will come with directions on how to consume the medicine and dosage for different age groups. Information related to the daily distribution of the kits should be sent to the state nodal officer, the order said.

Goa

Goa Health Minister Vishwajit Rane had on Monday said all people above 18 years will be given Ivermectin drug irrespective of their coronavirus status to bring down the number of deaths due to the viral disease.

Rane had said that people will be given Ivermectin 12 mg for five days as expert panels from the UK, Italy, Spain and Japan have found a statistically significant reduction in mortality, time to recovery and viral clearance in COVID-19 patients treated with this medicine.

Karnataka

On Tuesday, Karnataka Deputy Chief Minister and state's Covid task force head C N Ashwath Narayan said 10 lakh Ivermectin tablets have been procured and their supply will begin on May 14.

It has been further decided to procure 25 lakh tablets and make them available in all hospitals across the state, he said.

Medical experts, however, have questioned the efficacy of the five-day duration of the drug regimen announced by Goa government saying the treatment should not be given for a short period but ideally be continued till the pandemic is brought under control.

WHO's warning

Meanwhile, the World Health Organisation has warned against the use of Ivermectin to treat COVID-19.

“Safety and efficacy are important when using any drug for a new indication. @WHO recommends against the use of ivermectin for #COVID19 except within clinical trials,” Soumya Swaminathan, WHO Chief Scientist said in a tweet on Tuesday. However, regular use of the oral antiparasitic drug may significantly reduce the risk of contracting COVID-19, according to a review of available data by researchers who claim the medicine can help end the pandemic.

The research, published in the May-June issue of the American Journal of Therapeutics, is the most comprehensive review of the available data on Ivermectin taken from clinical, in vitro, animal, and real-world studies, its authors said.

Dosierung

Normalerweise müssen Ivermectin-Tabletten von Erwachsenen und Personen über 15 Jahren zweimal täglich drei Tage lang nach dem Frühstück und Abendessen eingenommen werden. Eine Person benötigt also sechs Tabletten und eine vierköpfige Familie 24 Tabletten. Daher wird jedes Kit zwei Dutzend Tabletten enthalten, heißt es in der Bestellung.

Rane hatte gesagt, dass die Patienten fünf Tage lang Ivermectin 12 mg erhalten werden, da Expertengremien aus Großbritannien, Italien, Spanien und Japan bei COVID-19-Patienten, die mit diesem Arzneimittel behandelt wurden, eine statistisch signifikante Verringerung der Sterblichkeit, der Zeit bis zur Genesung und der Virus-Clearance festgestellt haben .

Am Dienstag sagte der stellvertretende Ministerpräsident von Karnataka und der Leiter der Covid-Task Force des Bundesstaates CN Ashwath Narayan, dass 10 lakh Ivermectin-Tabletten beschafft wurden und ihre Lieferung am 14. Mai beginnen wird.

Es wurde ferner beschlossen, 25 Lakh-Tabletten zu beschaffen und in allen Krankenhäusern im ganzen Staat zur Verfügung zu stellen, sagte er.

Medizinische Experten haben jedoch die Wirksamkeit der fünftägigen Dauer des von der Regierung von Goa angekündigten Medikamentenschemas in Frage gestellt und gesagt, die Behandlung sollte nicht für einen kurzen Zeitraum erfolgen, sondern im Idealfall fortgesetzt werden, bis die Pandemie unter Kontrolle ist.

Warnung der WHO

Unterdessen warnt die Weltgesundheitsorganisation vor der Verwendung von Ivermectin zur Behandlung von COVID-19.

„Sicherheit und Wirksamkeit sind wichtig, wenn ein Medikament für eine neue Indikation eingesetzt wird. Die @WHO rät von der Verwendung von Ivermectin für #COVID19 ab, außer in klinischen Studien“, sagte Soumya Swaminathan, Chefwissenschaftlerin der WHO, am Dienstag in einem Tweet. Laut einer Überprüfung der verfügbaren Daten durch Forscher, die behaupten, dass das Arzneimittel zur Beendigung der Pandemie beitragen kann, kann die regelmäßige Anwendung des oralen Antiparasitikums jedoch das Risiko einer Ansteckung mit COVID-19 erheblich verringern.
